They found some germ in or around the factory, not on the equipment, not on the product, not in the product.

No idea on sickness or death if the unhealthy babies who also had formula. Did autopsies show it was that germ that caused or or something off with the formula caused it?

The sheer ignorance! Infant formula is one of the most regulated food products in the country. There's are reasons for this. 
1. To keep babies safe.
2. To ensure parents trust that the formula is safe.

It's not straightforward to change the regulatory regime, since the system is designed for safety...not resilience. We don't have a regulatory regime that makes it easy to take half-measures. Maybe we'll develop one, but it's hard to do it on the fly.[/quote]
