You rarely hear "Donald vs Clinton", "Clinton vs Donald", or "who will win, Clinton or Don"? If Clinton was President when the ACA was passed, would we be calling it "HillaryCare" instead of "ObamaCare"? And FYI, I'm a man. I don't think people are doing this on purpose but I do believe it has the effect of lowering her status by not calling her by her last name which is generally considered more respectful.
I think many are.
It is a common phenomena in our culture to refer to men by last names, women, children and serving staff etc. By first names. Sociolinguists love to chew this stuff up. Deborah Tannen spends a lot of thin in her books n this sort of thing. To call it misogynistic is a little extreme, though you modified it with unconscious which is probably more accurate. It does connect to gender roles. If you modify it in your own speech and go out of your way to be consistent it will eventually catch on.
