I agree with your gut feeling that something is not right. But "go to a doctor" may not be helpful when the issues are non-specific - this could be medical, mental, nutritional, some combo. She may need to see several doctors to figure it out, which will only happen if she's self-motivated to solve it. So just listening is what you can do for her. Reconsider that trip with her, because it would give you opportunities to listen and find out what is up. BTW, I think it can be fine to tell someone thet have BO. I mean, bosses have posted on here about having to tell employees, so you can tell your sister "dude, go take a shower before we go to dinner, you're getting stinky."[/quote]
