Our two favorite Tuscan towns are Sienna and Multipulciano.  If you are traveling to or from Rome, you can hire a car (we used and liked Daytrip) and stop in Multipulciano for a few hours on the way. Make lunch reservations in advance.

With the Duomo, you also want to get tickets to do the walk up to the top, if you can physically handle it and don't have young kids. the views are breathtaking and seeing the interior construction is pretty amazing.[/quote]
