At DOJ-tax I saw this countless times. OP, there is most likely a cashflow problem, and companies rob Peter to pay Paul. One of the early moves made out of desperation is almost always a failure to pay over employment taxes. It's possible that you just have a lazy manager who keeps failing to do the paperwork that gets people paid, but it is a lot more likely that there isn't cash to pay people and they are hiding that fact by pretending to have admin problems.
