It's not just once. It's perpetually failing health inspections. E coli, salmonella, norovirus, cross contamination is a regular thing at every place, the meat they use is not of good grade. Passes inspection but not the best considering you pay for good quality meat.

They hire illegal immigrants for kitchen duty. They regularly sexually harass the females working there. Big kitchen turnover but Chipotle does NOT resolve any claims. It's easier to just fire the accuser and the accused. Many waiting to take those spots.

Cleanliness happens only when an inspection is due. They are given a heads up when they will get one.  
I wouldn't feed my kids that filthy garbage.
