It is part of a Virginia Health Form which is required to be filled out and submitted. Note that in Virginia, day care/preschool/after care facilities operated by a church (as different from a private facility which rents from a church) do NOT need to be licensed or inspected by the Commonwealth. Licensing & inspection are optional for church-operated facilities. Similarly, I don't think very small in-home facilities with only a handful of children need to be licensed in Virginia.[/quote]
