As for the kid's size, a 5 year old can do a lot of damage if they hit someone in the back of the head, bite hard, head butt, hit with a heavy object, or even charge at an adult who is standing. This teacher was put in the cool down room with the child, suggesting he was already in full crisis mode. It's a terrible idea to put teachers in these rooms 1:1 with students like this.
