It's from how they grow in the lab. Worst case scenario is total implantation failure with ICSI. That means you have a retrieval, then no embryos at all. Then if they fertilize, but don't divide properly or have a lot of fragmentation or arrest/die before the embryos make blast, then that's a sign of bad quality too. Or they can make blast and fail to implant or result in chemicals/miscarriages, or make blast and all come back abnormal. An ugly, poorly graded blast can still be normal and result in a healthy baby, so I'd call that ugly, but not bad. Bad means you get eggs, but can't get embryos that can make a baby.

Total implantation failure could be a fluke, but twice and it probably means your eggs are just bad and donor eggs are your only option. Multiple cycles of not making blasts also means your eggs are probably just bad, though it's possible you could get EXTREMELY lucky like the PP above. Depending on insurance coverage and finances, most responsible RE's will say that 3 cycles is enough to know if your own eggs are going to work. You hear about people having success on their 5th cycle, but those are single digit odds and most people don't have the money or emotional bandwidth to keep putting your body through IVF cycles without any real chance of success.
