So when they see something, its s knee jerk reaction based on the orders they remember taking from physicians. There is little thought as to why the orders were being given. So, if they see 3 things, its a knee jerk reaction with 3 orders and they have no idea that the 3 orders are contradicting each other. They have no idea if and when an order is not appropriate in a situation. Ive seen it send quite a few people to the ER or worse.
