The garrote would have been after the fact to make it look like an abduction killing. If the brother inflicted the head wound (intentionally or accidentally) Patsy stepped in to cover it up. The garrote was fashioned using a paintbrush from Patsy's art set an the bristle part was discovered in its place (in a place unlikely discoverable by an intruder) while half of the stick part was never discovered.
