If asked, the doctor will provide a school report but only on request. Families are given a report containing diagnostic information and other details that are not on the school report such as weight, bp, visit history, etc. The school report contains information not found on the family report, like accommodations if needed. I have given the school report to schools before for my DC, but they always lose it and look confused when I bring a new one in. My DC sees his specialist once a year. I do not think that anyone at the private he attends ever reads the reports I give.[/quote]
