1) My theory is that Cersei (with the help of Qyburn) had the "little birds" (street kids) deliberately lure Pycelle and Lancel so that they would be killed in a particularly excruciating way -- she wants them to suffer more. Pycelle turned against her this past season when he was on the Small Council, and Lancel (who used to be her lover way back in Season 1) also turned against her when he joined the High Sparrow's army. Pycelle was lured by the little girl who told him (apparently) that King Tommen wanted to meet with him. Lancel was lured because he followed a little boy he saw acting suspiciously just outside the Sept - as the head of the Sparrow's goons, Cersei knew Lancel would follow the suspicious-acting kid.
