Part of the power of her act is the way she uses that child-like voice to deliver crude, even vile comedy. The whole point is to make the audience uncomfortable, and to challenge conventional (and often unconscious) expectations and notions about women. Many, many comedians perform in character. The voice and the crude commentary is very much part of her character. For more insight, I definitely recommend her book (Bedwetter). It was uneven for sure. I skimmed/skipped some parts, but others were quite thoughtful and thought-provoking.
