We saw a geneticist this year and he counseled us on if the school system ever says they will do a purge of his medical file, take it and don't tell them too much in the future.  (This was related to genetic issues, but I get that it is best not to have medical knowledge follow him from school to school.   He said "be vague" and he will send a note explaining my son has an issue and it is undiagnosed.
