We found that the beginning of every chapter was review, the middle of the chapter was at grade level, and the end of every chapter went just a bit farther than what they'd covered in school. We usually started in the middle of each chapter and would work to the end to strengthen skills. If my kids were struggling with a topic, we'd back up and do the sections early in that chapter.
