This most recent time I was trying to do 38+6 for logical reasons and was told no. However, I was just at VHC over a weekend post delivery. I was seen by the head of most departments on weekend days and had lactation consultants come on both a Saturday and Sunday so at least at VHC the concern about doctors was not reflected in my experience.
