If you are seeking a release from the club, the club ultimately needs to be involved in the release and MSYSA will look to the club for the ok and to submit the release form.  But, assuming you have completed your portion of the release form (https://www.msysa.org/assets/69/6/Player-Team_Status_Form_2020_-_REVISED_ver._A.pdf) and the club has been non-responsive, I'd send the form to registration@msysa.org, cc the club and note the club hasn't responded to you.
