Every state has a Katie Beckett waiver so that kids can be eligible for Medicaid based on disability rather than parents income. But many states have long waiting lists and the standard for disability is much higher than to get an IEP; it's essentially to prevent the need for institutional care.
+1. Katie Beckett waivers are designed to get around the requirement that a child be hospitalized for more than 30 days before Medicaid will stop looking at the parents' income to determine eligibility (which previously had resulted in kids who didn't qualify based on parents' income being left in hospitals for over 30 days when their parents could no longer afford to provide needed medical care at home). In essence, it's for children who have sufficiently significant medical needs that they could qualify for institutional care, but where care can be provided more cost-effectively in the home.
