In Home Physical therapists

Anyone have info/experience/contact information of physical therapists I can hire for my senior mom who can come home and work with her. She is visiting from another country, so no insurance or medicaid etc. I will be paying cash. Do physical therapists need a formal medical prescription?
Any idea on the approximate charges?

One hour is $180. We have used Brian Williams for a housebound family member and he has been excellent.
