question about test inclusion in the appeal package

Crunch time as I'm finishing the appeal package. I have some questions about building the package:

1. The form asks that a copy of the license be submitted when WISC is included in the packet. We have the report, but no copy of the license. We have the license number.

What did you submit in the package? Do we need to get a copy of the license?

2. Also --- did you submit the page with the actual test results, did you offer a narrative / interpretation of the scores or did you just list the results? Some of our tests (Language Processing asessment) don't have a summary sheet, as they inlined the result tables in the text. I need that to show a big difference from WISC in a key Narrative/ Verbal comprehension area. But not very sure how to physically insert that in my package.

Yes, you need to provide a copy of the license and yes, you submit the entire report (actual test results and the entire narrative).

You need to submit the entire report that was given to you by the dr. Also you should have a photocopy of her license. If YOU want to interprete the numbers then you can submit a seperate letter from yourself. That would count against the pages you can submit.

If you have the license number you can look it up online and print out verification of the license.
