Do you mostly sit and listen to what the teacher has to say or do you go in with an agenda of topics you want to cover? My DC is in 1st grade--I feel like it is the first "real" parent-teacher conference--and am curious about how others approach them...
Usually the teacher has examples of your child's writing, math, art, etc. to show you and discuss their progress, and then there is time for questions/concerns. They are usually on a fairly tight timeframe, and usually running late, so there isn't time for long tangents. They are usually only 15-30 minutes.
We have always brought in agendas we want to discuss with the teachers. Time is limited so make sure you cover the topics you are interested in. Also, starting in 2nd grade, some teachers invite the child to sit in for part of the conference and help present the portfolio of work. If needed, most teachers are open to scheduling other times to discuss important issues not covered as extensively during the conference.
