Are these inattentive ADHD signs?

Inconsistent grades and poor handwriting and spelling are symptoms, but I'm not sure about the rest. Mine can barely hold a conversation without medication, struggles to follow directions, gets lost in group instruction, procrastinates, loses everything, daydreams and talks to himself, and is sloppy and rushed in school when the subject matter isn't interesting.

Parts of this certainly sound like they could possibly indicate ADHD, but I would also talk to your pediatrician (or a developmental pediatrician) about getting a full workup to rule out other underlying cause of some of the physical symptoms (iron deficiency and interfere with sleep, for example, and lack of sleep can cause some of the behavioral things you're seeing). Our developmental pediatrician always pairs this sort of testing with a neuropsych to make sure physical causes aren't overlooked.

Sounds dependent on others. like he’s had people doing things for him too long. He can’t skip writing just because it’s hard, he needs more work on it not outs via voice to text.

Hard to tell op. Sorry that is hard! I did want to say that if the psychoeducational testing was at school, that is why you also don’t have a “diagnosis.” They don’t give diagnoses with that testing. They do what they did which is more just note where the child needs additional help in school. But for example if a child has dyslexia it won’t say that diagnosis specifically.

In your situation, I would personally consider a full neuropsych. The reason is that our psychiatrist (son has adhd but hyperactive so quite obvious!) explained that the times he recommends a neuropsych is when you think you might be missing something. That seems like your situation. You’ve tried the tutoring etc and still feel like something is missing. You’ll get a much more clear picture from a neuropsych. Wait lists can be long if you need to use insurance but if you can pay privately you can find places to get you in within a few months, sometimes even shorter. Anyway I know it is expensive so maybe there are other options but just a thought I had

OP, your kid sounds like my kid who was diagnosed with ADHD and Disorder of Written Expression.

TBH, IME, don't depend on special instruction at school - ours was low quality and not delivered with appropriate constructive feedback. Schools often say they are offering OG bug it is given by an instructor without appropriate training/qualifications or in a disparate geoup or with not enough frequency. In retrospect, I wish we would have delivered that instruction earlier and privately.

Has he been screened for celiac disease? That can cause skinniness, brain fog, etc. and not all docs will think to check for it

Did they find anything else like dysgraphia perhaps? That is sometimes present with ADHD inattentive type. You can't tutor these things away.

A year or so of OT moved my dysgraphic child's handwriting from godawful to merely bad. Switching him to cursive turned him into someone with basically normal handwriting, albeit on the slow end. So sometimes you can kind of tutor things away.
