Does he recognize when he needs to go/has gone? Many autistic children have delayed interoception and have difficulty recognizing their body sensations (when they have to pee, when they're hungry). OT can help with that.

Recognizing a need to go is not a prerequisite to toilet training. It’s helpful but it’s a waste of time at 5 years old to teach that instead of just toilet training them. You might need to use a watch or something but it’s fine.

Many years ago I worked in programs for people with disabilities. When I worked on the kid units, we toilet trained. It was really toilet timing. Here’s how we did it.

Because it was institutional, you controlled when they ate and drank and it was the same time every day.

When we were ready to train, what we did was to check their diapers every 15 minutes for a period of time so we could get their patterns. Once we figured that out, we’d direct them to the toilet at their time. This actually worked really well.

This can be a sign of Cerebral folate deficiency!
Is he taking high dose folinic acid or leucovorin?

It took us a long time at 4. We put the child on the toilet at wake up, before bed and every hour consistently. Eventually, we move away from pull-ups and let the accidents happen (covering the furniture, etc). We really had to push it. We only allowed the ipad on the toliet as an incentive.

Hm. My daughter trained just after 4 for pee and 4.5 for BMs. I took the diapers away and she would hold it for up to 8 hours. I would hold her on the toilet and she would scream, hit, cry and then finally release. It was unpleasant. About 2 weeks of that until she finally got over her fear. At 4.5, same thing, took diapers away (she would request one to poop in) and out of desperation she sat on toilet and got a princess dress as a reward. She was so proud of herself! She often says, “only babies use diapers now!” She is developmentally delayed and closer to 3.5 as a 5 year old. She could get an autism level 2 diagnosis but another psychologist said general developmental delay. Also she is my fourth child, my typically developing children had far more traditional potty training.
