No date in 2 months

We’ve been on and off for a year now, recently noticed we haven’t been on a formal date in 2 months.

I said something previously and he brushed it off, I feel uncomfortable saying something again.

Any advice? I guess saying it out loud is the obvious

"On and off" probably tells the story, unfortunately. Kind of sounds like you are more off than on. Or friends with benefits. Might be worth a conversation.

What's a formal date? I considered any time I spent with my now husband while we were dating to be quality time. Sometimes it was doing a workout class together. Sometimes it was dinner out. Sometimes it was taking the dogs for a hike. Sometimes it was watching TV. Sometimes it was doing laundry.

And how have you been on and off? Like you officially break up and then get back together again? If so, just be done for good. That's not a healthy relationship.

Agree, I don't like fancy restaurants so we rarely go.

Guys will always put effort into women they're actually interested in pursuing. He either considers you fully caught and doesn't think you require any more pursuit, or he's not particularly interested in you and will take what you have on offer but not look for more. Based on the "on and off" descriptor, it's likely option 2.
