I’m sorry I have no one specific to recommend, but there are people called Geriatric Care Managers who help with this. The ones I consulted years ago wire primarily in Montgomery County.
If the person is hospitalized the hospital social worker typically includes this as a part of discharge planning. Be aware that as in all professions, social workers come in a range and some of them are less concerned with a good fit than in getting the person off their “to do” list. |
I used a service called Owl Be There. It was recommended by the social worker working at the rehab place my dad ended up in after a hospital stay. He couldn't go home alone. He lived in Virginia and I lived in MD. They interviewed me and set up appointments with three facilities. They were all over Zoom. They helped me ask the right questions. They were very helpful.
It sounds almost like it is a vacation planner, in that they get a kickback from the facility. So, it was free of cost to me. |
We used Care Patrol (in NY, but they appear to be in the DMV, too) and I've heard A Place for Mom is good, too. The hospital social worker referred us to the Care Patrol contact,and later the two of them worked together to get my dad where he needed to be.
