WWYD 3rd failed FET - Exploratory Laparoscopic Surgery?

We have been TTC for 2 years and completed 4 IUIs, 1 ER and 3 FETs. We are 34 (f) and 38 (m) and unexplained although they suspect I have endo because of severe cramps. We have 1 untested embryo left (4BA) and I don’t know what to do - feeling so defeated. I’m considering getting an exploratory lap and excision if needed and try again for a few months before transferring my last embryo OR trying an unmedicated FET next month since my last FETs have been medicated with an immune protocol (prednisone, lovenox, LDN, baby aspirin). Would appreciate advice and also recs for surgeons on the DMV area. Thank you!

OP here - to clarify, I did 3 medicated FETs with the immune protocol. Unmedicated would be without immune protocol

Is this all with the same doctor? The one I went to would’ve ever done all of that (if I was paying OOP) with an unexplained diagnosis. You need a second opinion and more testing.

I’ve done all available testing and had 2 REs. RE isn’t recommending the surgery but I’m considering it after some research. They suggest doing another transfer.

Have you done a receptiva? Maybe do that first (its controversial) and treat with lupron if necessary before the last fet

I would either do an ERA or do another FET, but do it with medication and the immuno side of things. I don’t think dropping medication meant to sustain pregnancy are going to work in your favor.

This worked for me after numerous failed transfers. Why do you want a lap instead of lupron? Lupron can also reset the lining and get rid pf BCL6. Though as I said before, it's controversial. A lap is good if you want to get rid of the endometrosis and may help egg quality, but if you are not doing another retrieval, I'm not sure why it would be worthwhile.
