| Call the insurance. Find out what they need. My guess is a police report, and official statement from witness just in case. Police may press charges or do something with license plate. I’d start with insurance. |

Call your insurance broker to file a claim.
Call the person who left the note to thank them and let them know that your insurance company may be contacting them. The broker will then have someone from another department contact you to take a statement. The point is to ascertain who is at fault to determine which party needs to pay your deductible to fix the car. If you are going to fix the car out of pocket, you don't need to do anything but take it to a garage. |

I don’t know if you can trace the SUV owner’s name through their license plate number. But I know your insurance company and the police can. If they don’t leave you their contact info, they’re asking for an insurance claim against them and or a visit from the police. |
