What is the truth about the appeal process?

Some people say they only look at the new evidence and will not go over the original file.
Some people say they will add the new evidence with original file to review all over.
Which one is the real process???

+1. And how would they know that it's all new stuff if they don't look at the previous file to compare it to?

Per the George Mason program review (page 47):

Post Screening and the Appeals Process
Following the screening process, parents and schools are notified of the results.
For students who were identified as eligible, parents/or guardians make the final
determination of student enrollment. Parents, guardians, and the students themselves
who were determined to be ineligible may appeal the decision. The file is reviewed
again, the determination made, and is considered final for the particular academic year.
However, parents or guardians, the student, peers, a community member or teacher
may refer that student the following year.
