I'm still working on it. I got redirected once this morning when I first started, but not since then. I have to be able to reproduce the problem in order to fix it, so this is frustrating.

Google responded and has escalated to engineering to look into things further. They confirmed that ads such as this violate their terms of usage and would be removed if identified
Has anyone been redirected lately?

Argh! I had just decided the problem had been resolved and turned off my test setup. I guess I need to restart it and get back to work.

I also managed to get redirected a few minutes ago with my test setup. At the same time, I was contacted by Google support again. They have reports from other websites as well and are escalating the issue further. Because I just got redirected, I have full traffic captures and was able to provide them a ton of data. I think I have blocked everything I can. So, I don't know if Google's blocks are not working properly, I've not configured them correctly, or I missed something that I should have blocked.
As best as I can tell, two domains based in Germany, both of them with all of the contact info hidden, and both with no public presence on the web, are the ones doing this. Hopefully, Google will come up with a solution since I don't seem to be able.
