Ours did- but it was pertinent to a reason for why we wanted to get accepted so we sent it over. |
Yes. There’s usually a question asking if your child currently has one. And if so, they ask to see it. |
Yes. And if your kid needs accommodations, it’s really best not to try to hide that. If the school can’t or won’t meet their needs, you’re only hurting your kid if you try to deceive the school into accepting them. |

+1 Also, privates are not required to support either an IEP or comply with Section 504, unlike public schools. There are a few local privates which target students with special needs. |
